- It is necessary to transmit each Casualty Feeder Card to the relevant adjutant (S-1) as soon as feasible. When forms are received, the adjutant must inspect them for correctness and completion before forwarding them to the appropriate level of command headquarters for review and processing.
- The CMAOC must receive the first report within 12 hours of the event taking occurred since this is when the most serious consequences may occur. The DA Form 1156: What to Do and How to Fill It Out

- An asterisk (*) indicates that all of the fields on the form are necessary. When reporting a DUSTWUN, missing, or captured service member, be sure to mention the date, time, and location where the service member was last seen on the battlefield. The card must describe the missing individual, including his or her height, weight, age, eye color, scars, tattoos, and any other distinguishing characteristics that may be used to identify him or her. It is possible that the remains of a missing service member have been located and that they have been identified by a method of identification.
