The DA Form 2166-9-1 is governed by DA Pamphlet 623-3, Evaluation Reporting System, which is available online. The information included inside the form has been separated into five categories for your convenience. Administrative information, authentication, task descriptions, performance evaluations, and an appraisal of the officer’s potential for growth and development are all included in this section.
- It is necessary to collect the personal information of the assessed NCO for Part I, Administrative Data. This information should include the NCO’s name, e-mail address, social security number, information about the unit in which he or she serves and their rank, the reason for submission, the time period covered by this evaluation, the number of rated months, the number of nonrated codes, and the number of enclosures.
- Part II, Identification, contains information about the rater and the senior rater, among other things. These individuals’ names, social security numbers, and email addresses are provided in this section. The individual’s rank, service branch, and organization may all be included in the extra area. Both parties should sign and date the paper in the proper fields, using the right signature and date. If an additional review is required throughout the assessment process, an entire portion of Part II is devoted to the information needed to locate and contact a supplemental reviewer.
- Each of the following sections must be completed by the person in charge of conducting the evaluation: Part III, Duty Description, Part IV, Performance Evaluation, and Part V, Senior Rater Overall Potential. After all of the parts have been completed, the person who is conducting the assessment should present an overall grade of the NCO’s performance to the group. Additionally, an evaluation of the possibility for progression for the Sergeant NCO should be conducted, with the possibilities being “most qualified”, “very qualified”, “qualified,” or “not qualified.”
