DA Form 2166-9-1A – Army Pubs DA 2166-9-1A FillableDA Form 2166-9-1A, Non-Commissioned Officers Assessment Support Form, is a supplemental form that is used to collect information during the evaluation of non-commissioned officers (NCOs) in the United States Army who hold the rank of Sergeant or above (SGT). This form is a component of the DA 2166-9 Series, which also includes five additional rank-specific performance evaluation reports in addition to this one. It was released on November 1, 2015, by the Department of the Army in its most recent version, which is frequently referred to as the “DA Form 2166-9-1A.” (DA).

Counseling from a senior rater should be provided to the SGT NCO who is being assessed at least twice throughout the time of evaluation. After receiving counseling, the NCO is required to complete a form that includes a self-assessment of their work objectives and expectations, as well as a personal appraisal of their professionalism, performance, qualities, and abilities. If further remarks are required, a senior rater may supply them in addition to the self-evaluation.

The DA Form 2166-9-1A is governed by Army Pamphlet 623-3 (Evaluation Reporting System), which was published in November 2015 and is available online. The form is divided into four sections: administrative information, authentication, a task description (which is completed by the rater), and an examination of performance objectives and expectations (filled out by the Non-commissioned officer being evaluated).

  • In the first section of the form, the assessed NCO is expected to supply some of their personal information about themselves. Information about the individual includes their name, social security number (if applicable), rank, email address, and information on their unit. A statement regarding whether or not all Structured Self Development (SSD) and Noncommissioned Officer Education System (NCOES) criteria for the next grade or nominative assignment have been fulfilled is also important to provide (thus showing potential for promotion).
  • Identifying information about the rater and senior rater who will be completing the evaluation of the NCO’s performance and professionalism should be included in the second section of the form. The names, social security numbers, and email addresses of the raters are required for this section. It is required that both raters submit information on their rank, a branch of service, duty, and organization. Both raters must sign and date the paper in the relevant sections.
  • The rater is responsible for completing the third section of the DA Form 2166-9-1A (Duty Description) of the form. In the last section, the NCO is responsible for providing an explanation of their objectives and goals for the next assessment period.
