The DA Form 285-O Must Be Completed In Its Entirety

The Army Accident Investigations and Reporting manual, DA PAM 385-40, contains procedural standards and other information. The instructions for DD Form 285-O are included below.

  • To complete Section 1 (“Initial Review”), the reviewing official must identify themselves, their rank and title, and the name of their organization before indicating whether or not they agree with the findings and recommendations included in the report. If there are any discrepancies, they should be thoroughly explained.
  • A list of steps done, as well as any further suggestions for avoiding such incidents, should be included in Section 2 (“Army Headquarters Reviewing Authority Comments,” which is completed by the Army Headquarters Reviewing Authority). It is permissible for officials to use a letter-size piece of paper for extra information if the space supplied is insufficient. This material must then be attached to the form. After reviewing the findings of the accident investigation board, the Army Headquarters Reviewing Authority or its chosen representatives will submit their written consent or disapproval of the report. If there are any grounds for non-consent, this section should state those reasons as well as any suggested measures. The suggestions made by the accident investigation board or by subordinate reviewing officials will be taken into consideration by the Reviewing Authority for further consideration.
  • The comments made by the USACRC are included in Section 3 (“Department of Army Review”). After reviewing the previously provided suggestions, the USACRC will ensure that necessary action is done at the Department of Agriculture level.
  • To comply with Section 4 (Date of the Accident), the reviewing official must provide the date on which the accident happened.
