DA Form 3161-1 – Army Pubs DA 3161-1 Fillable – Request for Issue and Turn-In (Continuation Sheet), also known as DA Form 3161-1, is a form used to keep track of U.S. Army inventory that has been issued to a specific person or firm. For the purposes of this document, this particular form is a continuation sheet for the main DA Form 3161.
According to the United States Department of the Army (DA), the current version of the form – which is frequently referred to as DD Form 3161 – was made available on August 1, 2011, rendering all earlier copies outdated. Instructions For Completing DA Form 3161-1

- A request for either issue or turn-in must be labeled as such by the official who files the form, who does so by choosing the relevant option in the first box.
- In addition, the top line of the form needs the page number of each connected continuation sheet as well as the total number of pages utilized as a set of sheets. Other lines need the provision of the DODAAC, request, and voucher numbers, among others.
- The information on each item issued or returned must be entered into the main block of the form’s main block. It is necessary to number each entry, and each entry must include the stock number and a short description of each item, in addition to the unit of issue, quantities of the same item, supply action codes, unit pricing, and the dates on which the entry was made. The total cost of the goods on the list should be recorded at the bottom of the application form.
- To be included with the main DA 3161 document, the continuation sheet should be attached. A copy of the DA 3161-1 should be saved for future reference in order to guarantee an accurate recording of all products along the distribution chain.
