DA Form 4886 – Army Pubs DA 4886 Fillable – Issue-in-Kind – Personal Clothing Record (DA Form 4886), is a form that is used to keep track of the clothing that has been issued to members of the United States Army. When used in conjunction with the Army personal clothing record form, it is possible to maintain track of all provided things until they are returned or authorized to be kept.
The most recent version of the form, which is frequently wrongly referred to as DD Form 4886, was made available by the United States Department of the Army (DA) on July 1, 2014. Troops of the Reserve Components get their uniforms at the start of active service, while enlisted soldiers receive their uniforms during their first active duty for training purposes. When a soldier is dismissed or separated from active service, he or she must either return or retain the belongings, depending on whether or not the items are recoverable. Underwear and footwear are examples of non-recoverable products.
The DA Form 4886: How Do I Fill It Out?
Although the form may be submitted online, some of the elements must be filled in using an ink pen. This information contains the soldier’s name, rank, and the last four digits of his or her Social Security Number, among other things (SSN). The following are the instructions for DA Form 4886:
- If appropriate, please specify the size of each item you’re submitting. Insert information about successive garment inventory in Columns 1 through 15 of the spreadsheet. The item that was issued should be mentioned in the empty lines supplied at the bottom of the list if it is not included in the form.
- Please provide any extra information in the “Remarks” section of the document.
- Each issue must be followed by a date and signature at the bottom of the form, which is required for each issue. The decision on whether the soldier will be required to return the things or will be entitled to retain them is based on the length of their service and the terms of their discharge:
- If a service member is dismissed from active duty under honorable terms after more than three years of service, they are entitled to keep all of the personal belongings they had in their possession during that time. The unit commander is responsible for determining when the recovered objects should be turned in.
- If a service member is dismissed under honorable terms after serving three years or fewer on active duty, he or she is entitled to keep all of the clothes that were worn during that time.
- One ASU and component items may be retained by personnel who have served for six months or less on active duty and have been discharged with an honorable, general, or other than honorable discharge.
