The most recent version of the form – formerly known as DA Form 5305-R and often wrongly referred to as DD Form 5305 – was issued by the United States Department of the Army (DA) on July 1, 2020, replacing the previous edition. The DA Form 5305 enables these military members to create a complete family care plan that will benefit their whole family. This applies to the following categories:

  • Militaries who are separated from their spouses and have minor children (under the age of 19) or adult dependent family members who are unable to care for themselves are prohibited from serving.
  • Those serving in the military whose wives are unable to work owing to a condition;
  • Dual military spouses with underage children or adult-dependent family members who are unable to care for themselves are at risk of being separated.

  • Part I is concerned with the soldier’s family’s well-being and comprises declarations that must be accepted. By signing their name, the person submitting must attest that they agree with the terms of the agreement.
  • The guardian is designated in Section II. The soldier is required to submit the name and address of the guardians.
  • A spouse’s qualification and their command are included in Part III, which is reserved for dual-military couples exclusively.
  • Part IV of the form is allocated for the service member who is submitting the form as well as their commander. The only individual who has the authority to authorize DA Form 5305 is the unit commander.
