  • Before applying for any position, the applicant must first determine the sort of position for which they are applying.
  • Boxes 2 and 3 should include the necessary governing regulation number or circular number, as well as the application grade for reserve appointment.
  • Candidates for the Reserve Officers’ Training Corps (ROTC) are required to complete Box 4 (“Source of Application”), while applicants for appointment as warrant officers are required to fill out Box 5. Armed forces personnel who meet the necessary qualifications may enter up to three Military Occupational Specialty (MOS) codes in the applicable area.

The remaining five positions are up to selection by the applicant. Warrant Officer Aviators who want to be promoted to the rank of captain must indicate AV as their sole option on their commissioning application.
  • Boxes 7 through 20 ask the applicant to provide specific personal information about himself or herself.
  • The levels of military schooling are represented by boxes 21, 22, and 23.
  • Each statement in Boxes 24, 25, and 26 should be carefully read by the applicant, and each statement should be checked in the relevant box. Afterward, they must choose and complete all of the following sections by their branch of service, and then sign the form.
  • The completed Army DA Form 61, together with any supporting documentation, should be presented to a unit commander for consideration. The date on which they receive your paperwork is taken as the date on which your application was submitted.
