The DA Form 647-1: How Do I Fill It Out?
Personnel Accounting and Strength Reporting (Army Regulation 600-8-6) and Defense Administrative Procedures (DA Pamphlet 600-8), Military Human Resources Management Administrative Procedures, all of which provide further rules and information. It is recommended that a separate form be used for signing in and signing out. The following are the instructions for DA Form 647-1:
- In boxes 1 through 4, the soldier’s name, military organization, grade, and signature are all recorded for his or her convenience.
- Box 5 (“Date”) and Box 6 (“Time”) should be filled up with the proper date and time for each entry made in the database.
- The sort of movement is specified in the next field (“Action”). The box on the far right, under “Reason,” should include the reason for the relocation, which should be selected from the four possibilities provided.
- Soldiers who are leaving on transition leave should choose the PCS option and state the number of days they will be away in the comments section at the bottom of the page.
