DA Form 759-1 – Army Pubs DA 759-1 Fillable – Individual Flight Record and Flight Certificate – Army (Aircraft Closeout), also known as DA Document 759-1, is a form that is used to record flight time by flying duty and condition for each aircraft or flight simulator during the closeout period of the aircraft. DA Form 759-1 is only produced for aircraft that have been flown within the period covered.

The most current fillable version of the Army Publishing Directorate Form 759-1 is available for download below, or it may be accessed online at the Army Publishing Directorate website. The Department of the Army (DA) issued the most recent iteration of the form on July 1, 2018, making all earlier editions of the form outdated.

The Centralized Aviation Flight Records System (CAFRS) generates a distinct copy of the form for each aircraft and simulator indicated on the soldier’s consolidation worksheet, and each copy is kept on file by the military. For each Unmanned Aircraft Systems (UAS) operator and flight surgeon on DA Form 759-3, a separate DA Form 759-1 is created for each flight simulator, aircraft, and flight surgeon on DA Form 759-3. Filling Out DA Form 759-1: What to Do and What Not to Do

The Training Circular 3-04.11, Commander’s Aviation Training and Standardization Program, contains procedural rules as well as supplementary information on the subject of aviation training. The following are the instructions for DA Form 759-1:

  • Box 1 should include the number of the chronological sheet in question.
  • Boxes 2 through 4 of DA Form 759-1 are duplicated from Boxes 1 through 3 of DA Form 759-1 to provide the name, rank, and period necessary.
  • Box 5 is for defining the aircraft mission, design, type, and series of a flight simulator, as well as, if appropriate, the seat identification of the aircraft in question.
  • Boxes 6 through 12 (Columns A through J) include the total number of hours – in hours and tenths of hours – for all similar entries on the DA 759-2, as indicated by the flying duty and flight conditions symbols in the respective boxes.
  • Column K is allocated for the sum of the total number of hours across lines 6 through 12 for each of the flying conditions. Table DA 759-1 needs a column L with the sheet number from a prior DA 759-1 at the top, as well as the totals from Column M from the previous DA 759-1 for the same aircraft mission, seat designation, design, type, and series of a flight simulator.
  • Column M on lines 6 through 12 needs the new totals obtained by combining the figures in Columns K and L together. The hours in Column M are put together, and the result is entered on line 15. The hours in Column M are added together and reported on line 15.
  • Using Box 13, you may record the total number of hours spent in Columns A through J, expressed in hours and tenths of an hour.
  • On this line, you will enter the sheet number given in Column L, followed by the totals from line 15 of the preceding DA Form 759-1, which will be put in the respective columns on this line.
  • To complete Box 15, you must add the sums of lines 13 and 14, with the totals recorded in hours and tenths of hours in each case.
  • Columns A through J of Box 16 are filled out according to the consolidated DA Form 759-2 that corresponds to this particular box.
  • Line 17, Columns A through J, corresponds to the hours of immediate risk, as determined by the information included in the appropriate DA Form 759-2.
