DA Form 759-3 – Army Pubs DA 759-3 Fillable – Individual Flight Records and Flight Certificate-Army (Flight Pay Work Sheet), also known as DA Form 759-3, is a form generated by the Centralized Aviation Flight Records System (CAFRS) and used to keep track of the number of hours spent performing flights to calculate flight pays for military personnel. When a flight is done by a Rated Crewmember (RCM) or Nonrated Crewmember (NRCM), as well as conditional Aviation Career Incentive Pay (ACIP) recipients, the form is utilized as a temporary or consolidation worksheet. It is not necessary for Unmanned Aircraft Systems (UAS) operators.

DA Using Form 759-3 as a daily or monthly record is not necessary; a new form does not need to be utilized for each month. Filling Out DA Form 759-3: What to Do and What Not to Do

Box 1 should be kept blank unless otherwise specified. Your entire name is required in box 2.

Box 3 is for entering the military rank, “DAC” (Department of the Army civilian), “CIV” (civil workers of government agencies), or “CTR” (civilian employees of the Department of Transportation) (government contractor). If the option is not relevant, the box should be kept blank.

Box 4 is used to describe the period that is covered.

Box 5 asks for the sort of flying status you have: NRCM or Non-Crewmember (if applicable) (NCM).

Box 6 contains the effective date as determined by the flight instructions.

The next section of the form has a blank chart where you may input information about completed flights:

  • The dates of the flights are shown in Column A. To avoid confusion, the month and day should be placed on two different lines, with the day entered under the month and a blank line underneath it.
  • For months in which there have been no documented flying hours, the note “No Time Flown” must be placed across Columns B through D.
